For your convenience, we have attached important paperwork that may assist in expediting your needs. You may click on the links below to access and print these forms. Also, please be aware that we will ask for a photo ID and a copy of your insurance card for every visit with no exceptions.
Please arrive 15 minutes before your scheduled time. We highly recommend you complete your paperwork before you arrive. You can choose to fax or email it to us in order to save time or carry it in office with you. Patients need to bring in all their pill bottles for prescribed and over the counter medications they take and any old records.
Please arrive 10 minutes before to verify your insurance and update your demographic information and update your health history. Patients who take more than (3) medications please bring all your medications in their bottles at all your office visits.
Refill Request and Phone Messages:
We recommend you call your pharmacy for refill requests and give us 48 hours for response. Due to high call volume we suggest you use the patient portal and communicate with us. All urgent phone and portal messages will be returned on the same day and non-urgent messages will be returned within 24 hours.
If you arrive later than 10 minutes after your scheduled appointment time your appointment may be rescheduled depending on how busy the clinic schedule is for the day.
Cash Pay Patients:
We pride ourselves in offering competitive fee for service for those patients who do not carry any insurance. Please see bullet points down below for details.
New Cash Pay Patients $200
Follow-Up Cash Pay Patients $125
No Show Fees:
We require a 48 hour advance notice of appointment cancellation. A $100 fee will be applied to your account for short notice cancellations and no shows. Patients who repeatedly no show are subject to discharge from the practice. If you have been in the ER on the day of your appointment we can waive fee with proper documentation provided.
